Dental Excellence*
welcomes new patients. As a new patient you will need to register
with our practice and complete a comprehensive medical questionnaire.
From time to time we will also ask you to update this information,
particularly your medical details.
Why do we need your
personal information?
This
information is required to manage your dental records and account, and
to provide treatment that is appropriate for you in a safe manner.
All information is regarded as confidential and will not be made
available to any individual, entity or organisation without your
consent, in keeping with the provisions of the Privacy Act 1993 and the
NZDA code of practice on patient records.
Please complete this form
carefully, giving as much information as possible, even if something may
seem unimportant to you. Your dentist will be happy to review your
answers with you at your next visit and answer any questions you may
have.

us on our mobile telephones.

Fees
You can expect to pay NZ$66.00 for a routine consultation or
NZ$108.00 for a full examination
with two X-ray pictures. (Hygienist services and treatment items are
additional). After this initial diagnostic appointment, an estimate of
fees will be presented to you with your treatment plan. A written
estimate is readily available on request. Please feel free to discuss
fees at any time with either dentist or receptionist. Investing in
one's oral health is money well spent when the long term benefits of
good dental care and keeping one's teeth healthy for life are
considered.
Payment
Cash, Cheque, EFTPOS, Visa,
MasterCard, Activa and Bankcard are all accepted. Payment is expected at each
appointment.

Terms
of Trade
All
fees for goods and/or services provided are payable in full within seven
days of the date of delivery.
Any costs incurred by Khouri Miller Associates Limited in the
recovery of outstanding debts will be for the account of the debtor.
We
value your custom and our reputation and will work hard to preserve
both. If you have a problem with any aspect of your treatment, please
tell us. We will do everything we can to make you happy.
