We love welcoming new patients

Whether you're new to Hamilton, haven't been to the dentist in a while, or are simply looking for a dental practice that's the right fit for you and your family, we'd love to welcome you to Dental Excellence.

Our goal is to make your first visit as relaxed and straightforward as possible.

Before your appointment

All new patients are asked to complete our confidential New Patient Information Form before their first appointment.

This information helps us to:

  • Provide safe and appropriate dental care

  • Maintain accurate dental and medical records

  • Develop treatment plans tailored to your individual needs

  • Ensure we have the information we need to look after you properly

From time to time, we'll ask you to update your details, particularly your medical history, to ensure our records remain accurate and up to date.

Your privacy matters

All information you provide is treated as strictly confidential and is managed in accordance with the Privacy Act 2020 and the New Zealand Dental Council's standards for patient records and privacy of health information.

Please take the time to complete your form as accurately and comprehensively as possible—even information that may seem unimportant can sometimes be relevant to your dental treatment.

If you have any questions, your dentist or dental hygienist will be happy to discuss them with you at your appointment.

Returning your form

You are welcome to:

  • Bring your completed form with you to your appointment

  • Email it to us before your visit

  • Post it to our practice if preferred

Completing the form ahead of time allows us to spend more time getting to know you and discussing your dental care during your appointment.

Fees

You can expect to pay NZ$78.00 for a routine consultation or NZ$88.00 for a full clinical examination (X-Rays, hygienist services and treatment items are additional). After this initial diagnostic appointment, an estimate of fees will be presented to you with your treatment plan. A written estimate is readily available on request. Please feel free to discuss fees at any time with either your dentist, hygienist or receptionist.

Payment options

For your convenience, we accept:

  • EFTPOS

  • Visa

  • Mastercard

  • Cash

Payment in full is expected at the time of your appointment unless alternative arrangements have been made in advance.
